Transportation of a Minor by Adult Member Consent Form

In accordance with the US Center for Safe Sport guidelines and the Minor Athlete Abuse Prevention Policies (MAAPP) from the American Canoe Association, and per our club’s Bylaws, the following form must be completed by the parent/guardian of a minor athlete when one-on-one transportation from a sporting event (or club workout) is needed for a minor by an adult club member, including coaches.

(A copy of this form will be sent to the parent/guardian and another to the coaching staff.)

TO BE SUBMITTED ANNUALLY

I, as the parent/guardian of the Minor Athlete identified below, hereby authorize and consent that the Adult Member named below, can travel one-on-one with said Minor Athlete to and from all "In-Program sport activities" for the Ventura Canoe & Kayak Club for a time period of one year from the date of this consent.

I understand that my Minor Athlete or I can withdraw consent at any time. This form is being submitted to be in compliance with the American Canoe Association's Minor Athlete Abuse Prevention Policies (MAAPA).
